It's only an add-on module so it uses the same exact menu and pricing data as the standard POS does. All you would need to do is create a Premium layout, designing your desired interface. You would design where the buttons go, what images and text go on them and what the work flow looks like. Here's a link to a tutorial that describes that process:

No, because it's really not a pricing scheme, it's a workflow. Pick your special which has the full price associated with it, then it takes you to a page with your first choice of options. Pick one of those options, and it takes you to another page of options. Repeat this until the last option is added, and you are returned to the home page. All of the options would be 0.00 priced, and only the special itself would have a price on it. Only the options on the options pages would be available for the special in question.
If you have v8 installed, you can begin configuring the Premium POS Module right now without having the license. You can build your layout, and test it making sure that it behaves how you want it to. The only thing that it won't do without the license, is submit an order. Everything else is fully functional allowing you to set it up right away.
